Our Hospice Team

To provide hospice, we maintain a multidisciplinary team that includes:

  • Skilled Nurses, RNs and LVNs – evaluates the patient’s conditions during scheduled visits; he or she also coordinates the plan of care and guides the family over the course of implementing medication and pain management
  • Social Workers are friendly and skilled listeners who can offer emotional support and help with financial issues and planning.
  • Physicians collaborate with the patient’s primary physician and are available for pain and symptom management consultation, as well as to make house calls as needed.
  • Home Health Aides help with personal care, such as bathing and mouth care; they also can help with light cleaning, light laundry and occasional shopping.
  • Chaplains offer spiritual support.
  • Volunteers are specially trained in hospice and are important elements of the team; they are available when they are needed the most.
  • Hospice Dietitians assist the patient in achieving the best nutritional intake possible especially for the terminally-ill when eating can often be a challenge. Working with the physician, the dietitian suggests meal plans and food preparation techniques to make meals more appetizing for the patient.
  • Therapists: Occupational, Speech and Physical Therapy – therapists will help the hospice patient with adaptive devices as needed. The patient will receive teaching and instruction in energy conservation, mobility and fall prevention. The goal of therapy is to improve the quality of life for the patient during his or her remaining days.

All these individuals work cohesively to achieve quality hospice care for the client in the concluding stages of a terminal illness. We can also speak in various languages to improve communication with patients and family members:
